You will receive new patient paperwork and questionnaires that inform the doctor about your health history and current health issues. You will then have a private consultation with the doctor where she will go over your complaint(s) and conduct a physical exam including, but not limited to: spinal exam, vitals, neurological tests, range of motion, and muscle testing. After the examination you may be required to take any necessary x-rays. The next visit she will go through a report of findings where she will review your x-rays and examination results and discuss your diagnosis and treatment plan.

Yes! It is $25 for a sports physical which is due at the time of service, this is not covered by health insurance. Please have the form filled out and signed by the parent or guardian before arriving. If you did not receive a form from your school, you can download and print a copy of the form here.
